Mitral Valve Prolapse Syndrome Patient Developed Bigeminy during Laparoscopic Cholecystectomy

Abstract
A 43 year-old female patient with known mitral valve prolapse (MVP) underwent laparoscopic cholecystectomy due to cholecystitis. Preoperative laboratory results were within normal range except ECG, which presented unusual p axis, possible ectopic atrial rhythm. Echocardiography showed ejection fraction of 67% with mild mitral regurgitation due to MVP. At the operating room, ECG showed sinus rhythm. General anesthesia was performed and no adverse events were observed during the anesthetic management. At the end of operation, when anesthetic gas was turned off, bigeminy started to appear without showing hemodynamic instability. Patients with MVP may develop perioperative cardiac arrhythmias due to exacerbated autonomic system and altered catecholamine response.
